Prior to admission an assessment will be undertaken by the Management Team either at the Residents home or hospital in order to assess the potential resident's needs and suitability of placement.
Resident's possessions will be logged in their care plan. However, more sentimental or valuable items we would prefer to be kept in a secure location at the home unless the family specifically express that items remain with the resident. The home cannot take responsibility for such items on this occasion bearing in mind the cognitive skills of resident's with dementia and potential misplacement.
A comprehensive care plan will be generated for each resident which will cover all the needs and requirements of a resident. Families and representatives are encouraged to contribute to this plan to make it as comprehensive as possible including the resident's past history and preferences.
Personal furniture, memorabilia, pictures and electrical items suitable for a resident's room can be discussed with the management team prior to placement.
Communication with the home is important so several contact details would be appreciated to maintain the exchange of information that may be necessary with regard to wishes and healthcare decisions.
